Constant, Live-In, and 24-Hour Care

For seniors who require continuous support due to temporary, chronic, or terminal conditions, staying at home offers comfort and familiarity compared to assisted living facilities. At Caring First, LLC, our Constant Care, Live-In Care, and 24-Hour Care Services deliver compassionate, personalized assistance 24 hours a day, 7 days a week, tailored to meet the needs of housebound individuals or those who cannot be left alone. These services are ideal for those recovering from surgery, managing chronic illnesses like Multiple Sclerosis, or seeking to spend their final days at home with conditions such as advanced Alzheimer’s or cancer.

Our professional caregivers provide personal care, mobility assistance, household tasks, and companionship, adhering to Pennsylvania regulations and the U.S. Fair Labor Standards Act (FLSA). We offer medication reminders (prompting or observing self-administration only), limited financial assistance (e.g., sorting mail with consent), and incidental transportation using the client’s vehicle under strict policies. Live-In Care ensures consistency with fewer caregivers, while 24-Hour Care provides shift-based support for constant availability. Clear service agreements detail on-duty periods, breaks, and sleep, ensuring compliance and peace of mind for families.

Live-In Care vs 24-Hour Care

We offer two flexible options to meet your loved one’s needs, each with distinct advantages:

Live-In Care

Live-In Care involves at least two Home Care Assistants (HCAs) assigned to provide 24/7 support, typically with one covering up to 5 days per week and another covering the remaining days. This option is ideal for clients who don’t require full-time overnight attention or turning during the night. Key features include:

  • Consistency: Fewer caregivers foster stronger bonds and familiarity, especially beneficial for clients with dementia.
  • Cost-Effective: Generally less expensive than 24-Hour Care due to fewer staff hours.
  • FLSA Compliance: Caregivers receive a 10-hour workday, 8 hours of sleep (at least 5 uninterrupted), 3 hours for meals, 3 hours for breaks, a private bedroom, and meals provided by the client, per service agreements.
  • Limitations: Additional charges apply for full-time overnight care, and caregivers are not permitted to use client property (e.g., phones, TVs) or have personal visitors.

24-Hour Care

24-Hour Care involves two 12-hour shifts or three 8-hour shifts, with different HCAs assigned to each shift, none of whom live in. This option ensures a caregiver is awake and available at all times, including overnight. Key features include:

  • Constant Availability: Ideal for clients needing full-time overnight supervision, with options for “sleeping visits” at reduced rates or full-time availability.
  • Team Approach: Two or three HCAs are assigned to minimize disruption and maintain consistency.
  • Flexibility: Shift-based care accommodates complex needs, such as frequent nighttime assistance.
  • Considerations: More caregivers may cause confusion for some clients, and costs are higher than Live-In Care.

Our Commitment to Compliance

We adhere to Pennsylvania regulations (55 Pa. Code Chapter 611) and the U.S. Fair Labor Standards Act to ensure high-quality, ethical care:

  • Caregiver Well-Being: Live-In caregivers receive a private bedroom, meals, 8 hours of sleep (5 uninterrupted), and designated breaks, as required by FLSA, with schedules documented in service agreements.
  • Professional Boundaries: Caregivers are prohibited from using client property (e.g., phones, TVs, cars) or having personal visitors, per agency policy, with regular audits to ensure compliance.
  • Medication Reminders: Our Medication Management Policy limits caregivers to prompting or observing self-administration, with no dispensing or application, and requires documentation of reminders and client responses.
  • Transportation Policy: Incidental transportation using the client’s vehicle requires valid licenses, insurance coverage, and client consent, with documented usage to prevent misuse.
  • Financial Assistance Limits: Caregivers assist only with sorting mail or organizing bills, with client or legal representative consent, and are prohibited from managing funds directly, per agency policy.
  • Training and Oversight: Caregivers receive initial and annual training, with documented competency checks for personal care, mobility, and other tasks, ensuring compliance with state regulations.
  • Documentation and Referrals: We document all care delivery, including tasks, client condition, and concerns, and refer clinical needs (e.g., medication changes) to healthcare providers.
